Costs & Insurance Codes
CPT-4 PROCEDURE CODES
Our office policy requires that all procedures be prepaid. We do not bill
insurance carriers. Amen Clinics, Inc has opted out of Medicare, Medi-Cal, and/or
Medicaid. Should you be interested in seeking reimbursement from your insurance
carrier; our office will provide a receipt which will include codes and
descriptions required by your insurance. Since our doctors and therapists are
not affiliated with any insurance plan your insurance may or may not cover our
services and fees. You will need to contact your insurance to confirm your type
of coverage and mental health benefits, if any.
