Cancellation and Rescheduling Policy

Depending on the type of evaluation, a deposit of $500 – $1,500 is required to secure an appointment.

  • If you cancel or reschedule an Evaluation within three (3) business days of making the appointment, there is no cancellation or additional fee to reschedule if you notify us by telephone and during regular business hours (M-F 6:00am-6:00pm, Sat 8:00am-12:00pm PST).
  • If you cancel an Evaluation after three (3) business days of making the appointment, we cannot not provide any refunds. Your deposit will be credited towards any future Amen Clinic appointments.
  • If you need to reschedule any scheduled appointments after the initial evaluation, a two (2) business day notice is required.
  • The remaining balance of your evaluation is due at the time of the first appointment.
  • If you are a “no-show”, we will not provide any refunds.

Prices subject to change until they are paid in full. All major credit cards accepted. We do not accept cash as a method of payment.

Care Credit Disclosure

No interest if Paid in Full within the Promotional Period

On qualifying purchases of $200 or more made with your CareCredit credit card account at enrolled provider locations. Interest will be charged to your account (at the rate of 26.99% for new accounts) from the purchase date if the promotional purchase is not paid in full within the promotional period.

Reduced APR and Fixed Monthly Payments Required Until Paid in Full**

On qualifying purchases made with your CareCredit credit card account at enrolled provider locations.

  • Purchases of $1000 or more are eligible for a 24, 36, or 48 month offer with a 14.09% APR.
  • Purchases of $2,500 or more are eligible for a 60 month offer with a 16.90% APR.

Interest will be charged on promotional purchases from the purchase date at a reduced 14.90% APR on purchase with 24, 36, 48 months promotional financing and a 16.90% APR on purchases with 60 months promotional financing. Fixed monthly payments are required until paid in full and will be calculated as follows: on 24-month promotions 4.8439% of initial promotional purchase amount: on 36 month – 3.4616% of initial promotional purchase amount; on 48-month promotions – 2.7780% of initial promotional purchase amount or on 60-month promotion – 2.47999% of initial promotional purchase amount. The fixed monthly payment will be rounded up to the next highest whole dollar and may be higher than the minimum payment that would be required if the purchase was a non- promotional purchase. Regular account terms apply to non-promotional purchases except the fixed monthly payment will apply until the promotion is paid in full. For new accounts, the regular Purchase APR (interest rate) is 26.99%; Minimum Interest Charge is $2. Existing cardholders should see their credit card agreement for their applicable terms. Subject to credit approval.
